Care Coordination with a Difference

Care management services identify, coordinate and monitor needed resources like health care, personal care and supportive services. These services are especially valuable when family members live out-of-town, or simply can't do it all.

Care management services have wide-ranging benefits that include:

  • Instituting the full range of support needed to remain living comfortably and safely at home.
  • Addressing issues before problems arise or begin to threaten health, safety, independence, comfort or dignity.
  • Coordinating services with doctors and other care providers.
  • Reducing the caregiving burden on local family members.
  • Keeping family members informed about changing needs, while providing a local contact for out-of-town families.
  • Recommending and coordinating alternative living arrangements if needed.

How Care Management Works

Care management services begin with a family consultation by a professional Care Manager. The consultation includes a comprehensive assessment of a client's and family's needs, and development of a plan for the client's care and support.

The family may then choose to initiate and oversee care provision on their own, or purchase ongoing care management services that can include:

  • Arranging and monitoring care and support.
  • Contacting the client and family regularly.
  • Adjusting care and support based on changing needs and family decisions.
  • Finding and coordinating a client's move to an alternative living situation (if needed).

Ongoing care management services can be especially reassuring to long-distance family caregivers, ensuring that care is knowledgably coordinated and everyone involved is alerted if problems arise.

Increasingly, on-call care management services are requested by local family caregivers taking a break from caregiving or planning for emergencies. By purchasing on-call care management services, a family gains access to a local, professional Care Manager who can handle emergencies and special needs when a primary family caregiver is out-of-town for business or vacation, or suddenly becomes unavailable.

A client or family may also benefit from Home Care or Personal Assistant Services in addition to Care Management Services.

Care Management Staff

Our professional Care Managers are experienced resource experts trained in gerontology, social work and/or nursing who specialize in meeting the challenges of aging. Their involvement can be short-term or long-term, depending on the level of support required.
