- SAN MATEO: 122 SECOND AVENUE, SUITE 214
Wednesday from 8:30 to 2:30
Thursday from 8:30 to noon
- PALO ALTO: 3335 BIRCH STREET
Monday and Tuesday from 1:00 to 4:00
Wednesday and Friday from 9:00 to noon
In order to process your application, please bring with you:
- Social Security Card
- Valid Driver’s License or California ID Card
- Proof of citizenship, or documentation of ability to work in the United States
- 3 references about your experience as a caregiver (with names & phone numbers)
- Any certification cards (CNA, CPR, HHA, First Aid, etc.)
- Health exam and TB test report if completed within the last 6 months (this can be arranged later as needed)
- Proof of auto insurance
