At Older Adults Care Management, each of our high quality services is custom- designed to meet the unique needs of clients and their families.

State-licensed home care services provide custom solutions to fit most needs and budgets. Options include temporary or permanent care, part-time or full- time care, and care for people with dementia, stroke and other special needs. Our skilled, experienced care providers can live-in or outside of the home. Many have received specialized dementia care training from the Alzheimer’s Association.

Care Management services offer a professional, comprehensive assessment of the care and support needed, development of a plan for care, and coordination of outside services needed for clients to remain living comfortably at home. Care management is especially helpful for monitoring care and supportive services, and providing peace-of-mind to family members helping from a distance.

Personal Assistant services offer bonded support with bill paying and banking, and help with other personal needs like correspondence and filing. Our staff can arrange any number of personal services like haircutting, nail care or pet care, and can serve as a companion at restaurants, movies, museums, concerts and family events.

Out-Of-Home Care can be scheduled to give additional one-on-one support to loved ones residing in an assisted living facility, rehabilitation center, or long- term care facility.
